1. Agreement
These Terms of Service (“Terms”) govern your access to and use of itsmentalwellness.com (the “Website”), operated by Heidi Reed, MSN, APRN, PMHNP-BC, doing business as Intersect Mental Wellness in Arizona and Nevada (the “Practice,” “we,” “us,” “our”). By using this Website, you agree to these Terms. If you do not agree, please do not use the Website.
2. The Website is not medical advice
The content on this Website — including information about psychiatric conditions, treatments, services, and the Practice itself — is provided for general educational and informational purposes only. It is not medical advice. It is not a substitute for diagnosis, evaluation, or treatment by a licensed healthcare professional.
Reading content on this Website, submitting the contact form, or exchanging email with the Practice does not:
- Establish a provider-patient relationship.
- Create any clinical, diagnostic, or treatment obligation.
- Replace the need for a proper psychiatric evaluation.
A provider-patient relationship is formed only after you complete the intake process (including informed consents and the initial evaluation) and we agree in writing to enter a treatment relationship.
3. Emergency disclaimer
This Website is not monitored 24/7. Messages submitted through the contact form or sent by email are generally reviewed on the next business day. Never use this Website or any communication channel associated with it to report or respond to a crisis. In an emergency:
- Call 911 for immediate danger or medical emergency.
- Call or text 988 for the Suicide & Crisis Lifeline, available 24/7.
- Go to the nearest emergency department for urgent psychiatric evaluation.
4. Who can use this Website
This Website is intended for adults (18 and older) and for parents or legal guardians of children seeking psychiatric care. If you are a minor, please use this site only with the knowledge and supervision of a parent or legal guardian.
5. Scope of practice & licensure
The Practice is operated by Heidi Reed, a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) licensed by the Arizona and Nevada Boards of Nursing. The Practice provides outpatient telehealth psychiatric care only to patients physically located in Arizona or Nevada at the time of service. You have the right to verify licensure at azbn.gov or nevadanursingboard.org.
A PMHNP is a registered nurse with advanced graduate-level training; a PMHNP is not a physician. Clinical care is provided within the PMHNP’s licensed scope of practice under Arizona or Nevada law.
6. Telehealth
The Practice offers clinical services via telehealth. Telehealth services are available only to patients who are physically present in Arizona or Nevada at the time of the visit. Patients must provide informed consent specifically for telehealth before any telehealth visit. The following limitations apply:
- Technology failures (video freezes, connection drops, etc.) may interrupt a visit and occasionally require rescheduling.
- Telehealth is not appropriate for every clinical situation. The clinician may refer you to a more appropriate level of care.
- If a patient in a telehealth visit experiences a crisis during the session, the clinician will follow emergency protocols, which may include contacting emergency services at the patient’s location.
7. Communication & response times
The Practice is not an emergency service and does not provide 24/7 on-call coverage. Non-urgent messages (via the patient portal, email, or phone) are typically returned within 48 hours. For any question or concern that cannot wait, use the emergency resources listed above.
Email is not a secure channel for Protected Health Information. Once a provider-patient relationship is established, clinical communication should occur through the secure patient portal.
8. Appointments, payment, and cancellation
Detailed fee, payment, and cancellation terms are described on the pricing page and incorporated into the patient agreements.
In summary:
- This is a cash-pay only practice. Payment is due at the time of service, and a valid credit card must be kept on file.
- Under the federal No Surprises Act, uninsured and self-pay patients have the right to receive a Good Faith Estimate of expected charges before scheduled non-emergency services.
9. Intellectual property
All content on this Website — including text, images, logos, designs, and page layouts — is the property of the Practice or licensed to the Practice, and is protected by United States and international copyright and trademark law. You may view and use this Website for personal, non-commercial purposes only. You may not copy, reproduce, republish, distribute, or create derivative works of any Website content without prior written permission from the Practice.
10. User conduct
When using this Website, you agree not to:
- Misuse the Website or interfere with its operation.
- Attempt to access any restricted area, system, or data.
- Submit false, misleading, or abusive content through the contact form.
- Use the Website to harass, threaten, or defame any person.
- Violate any applicable law.
11. Third-party links & resources

12. No warranty

To the fullest extent permitted by law, the Practice disclaims all warranties, express or implied, regarding the Website — including warranties of merchantability, fitness for a particular purpose, accuracy, or non-infringement. We do not warrant that the Website will be uninterrupted, error-free, or free of harmful components.
13. Limitation of liability
To the fullest extent permitted by Arizona or Nevada law, the Practice will not be liable for any indirect, incidental, consequential, special, or punitive damages arising out of or related to your use of this Website.
Nothing in these Terms limits any liability that cannot be limited under Arizona or Nevada law, including any liability for gross negligence, willful misconduct, or professional malpractice.
14. Indemnification
You agree to indemnify and hold harmless the Practice, its employees and contractors, from any claim or demand arising out of your breach of these Terms or your misuse of the Website, except to the extent caused by the Practice’s own negligence or willful misconduct.
15. Governing law & venue
These Terms are governed by the laws of the States of Arizona and Nevada, without regard to conflict-of-laws principles. Any dispute arising out of or related to these Terms or the Website will be resolved exclusively in the state or federal courts located in Arizona or Nevada, and you consent to the jurisdiction of those courts.
16. Changes to these Terms
We may update these Terms from time to time. When we do, we’ll post the updated version with a new effective date. Significant changes will be communicated prominently. Your continued use of the Website after changes take effect constitutes acceptance of the revised Terms.
17. Severability & entire agreement
If any provision of these Terms is found to be unenforceable, the remaining provisions will remain in full force. These Terms, together with the Privacy Policy, the Notice of Privacy Practices, and (for patients) the patient agreements signed at intake, constitute the entire agreement between you and the Practice regarding your use of this Website.
